If the student's Former School is an out of state/country school, select "Non-NJSIAA School" as the Former School. You will be prompted to fill out additional information for that Former School. Please try your best to locate a valid email address for the school as the Former School Form will be sent this this email address.
Email deliverability from a non-email platform will never be 100%, so we suggest manually emailing this form as well.
